You assume that I only practiced medicine in the USA You assume that US hospitals run like businesses (private insurance companies, most hospitals don’t) You assume doctors, especially subspecialists grow on trees.

You want it because your patients are not well served by general practitioners, and providing 3rd parties the whole context of care they need is difficult. That’s fine.
